Subdivision Reporting Forms Following are links to annual Activity Report Forms, to be completed by community chairs: Sections/Societies (rtf) (pdf) Chapters (rtf) (pdf) Student Chapters (rtf) (pdf) Fora (rtf) (pdf) Activity Reports may be filed in one of two ways: Download the form in rich text format (RTF)and simply fill it in using MS Word.  Then e-mail the completed report as an attachment. During subsequent reporting cycles you will only have to fill out fields that have changed.
